Angioplasty and Stents Explained: What Really Happens During a Coronary Intervention

A clear, step-by-step guide to coronary angioplasty and stenting — from arterial access and imaging to recovery and ongoing heart care.

For many patients diagnosed with a blocked coronary artery, the recommendation to undergo "angioplasty and stenting" can feel unfamiliar and, understandably, a little daunting. Understanding what the procedure actually involves — and why it works — can go a long way toward easing that uncertainty.

What Is Angioplasty?

Angioplasty, more formally known as Percutaneous Coronary Intervention (PCI), is a minimally invasive procedure used to open a coronary artery that has become narrowed or blocked by plaque. Unlike open-heart surgery, angioplasty does not require the chest to be opened. Instead, it is performed through a small puncture, most commonly in the wrist artery, occasionally in the groin, using thin, flexible catheters guided to the heart under live X-ray imaging.

Step by Step: What Happens During the Procedure

  1. Access — A small puncture is made in the wrist or groin artery, and a thin sheath is inserted to allow catheters to be passed through safely.
  2. Angiography — A catheter is guided to the coronary arteries, and contrast dye is injected to map the exact location and severity of any blockages under live imaging.
  3. Crossing the blockage — A very fine guidewire is carefully advanced across the narrowed or blocked segment of the artery.
  4. Balloon angioplasty — A balloon-tipped catheter is passed over the guidewire to the blockage and inflated, compressing the plaque against the artery wall and widening the vessel.
  5. Stent placement — In most cases, a stent — a small, expandable metal mesh tube — is then positioned and deployed at the site to hold the artery open and provide a scaffold that supports long-term blood flow. Most modern stents are drug-eluting, meaning they slowly release medication that helps prevent the artery from re-narrowing.
  6. Confirmation and closure — A final angiogram confirms that blood flow has been restored, after which the catheters are removed and the puncture site is closed.

The entire procedure typically takes between 30 minutes and a couple of hours, depending on the complexity of the blockage, and most patients are able to go home within a day.

Why Imaging Matters

In more complex cases — such as heavily calcified arteries, blockages at branch points, or long-standing total blockages — additional tools may be used to improve precision and outcomes. Intravascular imaging techniques such as IVUS (intravascular ultrasound) and OCT (optical coherence tomography) allow the interior of the artery to be visualized in fine detail, guiding more accurate stent sizing and placement than X-ray imaging alone. For arteries with significant calcium build-up, specialized techniques such as rotational or orbital atherectomy, or intravascular lithotripsy, may be used to modify the calcium before stenting, allowing the artery to expand properly.

Emergency Angioplasty for Heart Attacks

When angioplasty is performed urgently during an ongoing heart attack — known as Primary PCI — the goal is to restore blood flow as quickly as possible, ideally within the first hour or two of symptom onset, to limit permanent damage to the heart muscle. This time-critical approach has become the standard of care for eligible heart attack patients and has significantly improved survival and long-term heart function.

After the Procedure

Recovery from angioplasty is generally quick, but ongoing care matters just as much as the procedure itself. Patients are typically prescribed antiplatelet medication for a defined period to prevent clot formation within the stent, alongside other heart-protective medications. Attention to lifestyle factors — diet, physical activity, and smoking cessation — along with regular follow-up, remains essential to protecting the treated artery and preventing new blockages elsewhere.

Angioplasty and stenting have transformed the treatment of coronary artery disease, offering a minimally invasive way to restore blood flow with a relatively short recovery. Understanding the mechanics of the procedure can help patients approach it with greater confidence and clarity.
